Case Management


We know that navigating the healthcare system can be overwhelming. Our Case Management program is designed to work in partnership with you, your doctors, and other providers to support you in your ongoing healthcare needs, so you can rest assured that your voice will always be heard and your health will be taken care of.

We can help!

You will be assigned a Case Manager to work closely with you and your primary care physician. We want to ensure that you receive the most appropriate level of care and services and that you take full advantage of the benefits available to you through your health plan. Your Case Manager can:

Teach you about your condition

Guide you in making a treatment plan

Help you get medications and care

Set up referrals and doctor visits

Work with all members of your health care team

Patient Safety

We provide comprehensive care with the intended highest level of patient safety, based on the anticipated risks that the patient might encounter across several factors (drug interactions, frailty, over-the-counter medication, among others), preventing mispractice

Risk management

You may have your own healthcare-risk assessment to address the right approach, accordingly. Being able to have a general sight of your health status gives you a chance to receive the care you need in a timely way. 

Physician Advisor

We help you by approaching your needs on health issues to the right physician so you get the proper care.

How It Works

The 4 Levels Of Medical Case Management

1. Intake and Referral

The physician case manager will receive all the patient's information: documents, medical records, including past and current health conditions, and more. The patient's immediate needs are identified and trust will begin to build during the information gathering process.

2. Needs Assessment

We review the patient's records and the information collected. We identify his problems, challenges, risks, and circumstances that led to the injury or illness. Co-morbidities and socioeconomic barriers to effective recovery are considered. The physician case manager makes appropriate contacts to help find the most appropriate and cost-effective care solutions.

3. Care Planning

Once the short- and long-term goals of care have been established, the physician case manager will develop a comprehensive plan of care that addresses the patient's problems and needs. The plan details the treatments and services needed.

4. Monitoring and Follow-Up

The physician case manager will review and evaluate the effectiveness of the patient's case plan, the effect on the patient's health status, knowledge of the treatment regimen, and the results of medical procedures and interventions. If necessary, re-evaluations and modifications are made with additional recommendations for care if needed. Regular progress reports will be provided that include meaningful updates on the client's comfort, medication intake, and any problems or concerns and changes to the plan of care and schedule. Follow-up is also provided from the time of discharge until transfer to another facility or home care.
